Personal Health Record integration with Electronic Medical Record

First, we need to define terminology. A personal health record (PHR) is usually kept by an individual – a potential patient. An electronic medical record (EMR) or electronic health record (EHR) is usually populated by a physician's office or healthcare provider such as a hospital or nursing home.


Rarely does the personal health record communicate with the electronic medical record being kept at the physician's office.  Why not, you ask? For one thing, patients do not assume responsibility for their own health care. They abdicate that responsibility to the physician. In fairness, the physician often fails to communicate all the pertinent information to the patient. This results in a gap that Lifesaver App is attempting to fill.
